Job Details Job Location : 621 W MADRONE - ROSEBURG, OR Position Type : Full Time Education Level : 2 Year Degree Salary Range : $45,000.00 - $61,000.00 Hourly Job Shift : Day Description Adapt Integrated Health Care is looking for a Case Manager to support their SUD services! We Offer: 100% paid premiums for full family medical insurance Waiver cash option Dental and Vision insurance available Generous sick and vacation time accrual rates; accrued time rolls over year-to-year. 5% 401k employer contribution Life insurance policy Employee Assistance Program Clinical supervision toward licensure is available. The SUD Case Manager provides case management, needs assessments, and care coordination to substance abuse and co-occurring clients, assisting those clients in reaching self-sufficiency. Primary Responsibilities: Collaborates with the client on a needs assessment to identify areas of focus Assists with recovery plan development and implementation, coordinating with the primary counselor and other key staff Investigates resources and provides referrals to appropriate community agencies Integrates client supports (family, social, community) to help remove barriers for clients Coordinates referrals, within the ASAM level of care, to residential facilities to affect a smooth transition between services Documents in a timely and appropriate manner each intervention and coordinates information with the client's support team Attends staff meetings and development trainings as scheduled Participates in clinical supervision, including QA review of documentation and personal goal tracking highlighted in yearly evaluation Qualifications Required Qualifications: High School Diploma or equivalent required.
