Exciting News: We are growing our in-house therapy program. A thoughtful initiative aimed at enhancing our therapy services within the home health sector. As part of our team, you will have the opportunity to contribute to this program, helping it to evolve to better serve our clients and the community. Roles and Responsibilities: Conduct patient assessments and establish physical therapy treatment goals. Implement individualized therapy programs designed to restore, reinforce, and enhance performance. Facilitate the learning of skills and functions essential for adaptation and productivity. Contribute to the development and evolution of our "Pinnacle Therapy Excellence" program. Document and record the patient's condition and educate patients and families in an appropriate physical therapy method. Requirements: Valid Physical Therapist license in the state of Texas. Minimum of 1 year of physical therapy experience preferred; home health experience is a plus. Excellent communication and patient care skills. Ability to work independently and manage time effectively. Strong interpersonal skills, problem-solving skills, and ability to work in a team.
