Job Title: Phase II Coordinator Summary of Job Duties : The Phase II Coordinator is responsible for assessing, coordinating and providing services for Cumberland River RHOAR Phase II clients. Qualifications : To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. High School Diploma is required, however college experience or CADC is preferred. Knowledge of women’s issues in recovery is helpful. If in recovery, 2 years of continuous abstinence from drugs/alcohol required. Submit to and pass a criminal record check and a drug screening test. Hours : Monday through Friday 8:00 am until 4:00 pm Evening and weekend hours may be required. This position reports directly to the Program Director. Essential Duties and Responsibilities: Include the following: Fulfills the responsibilities of the Assistant Staff when necessary: Coordinates, plans and implements the program content of Peer Mentors Provides administrative and secretarial support which may include typing correspondence, letters, speeches, presentations, and other relevant mediums. Responsible for collection of RCOS data from clients and submittal to UK Daily Headcount Assist the driver/monitor with client transportation Prepare and maintain a list of agencies and contact information for referrals Daily client room searches Daily client drug screens Attend Community Facilitates Phase II Community Coordinate with courts, judges and referral agencies to provide services Works regionally with police, medical personnel, and other community service agencies and individuals to provide and promote optimal services to clients Acts a s a liaison with and makes client referrals to Vocational Rehabilitation to address employment/educational needs Acts as a liaison with Section 8 Housing to ensure clients are aware of the requirements of reporting changing and new information to the appropriate office Works with after-care plans and clients regarding housing, education and employment Provides general case management services to clients Initiates and maintains complete and up-to-date client records (client files, shift logs, incident reports, discharge summaries, etc.) Assures maintenance of client information in secure files Develops community resources for the program which meet individual client needs Serves as a member of the management team, which creates and maintains a consistent and nurturing environment for all clients Collaborates with other staff in program development and client consultation. Provides feedback and education to all clients in collaboration with other members of the management team Assists in developing and adhering to all policies and procedures Teaches Life Skills and open classes Transport clients as needed Provides daily supervision to ensure accurate, complete and up-to-date records are kept and all clientele are receiving appropriate care Coordinates with RKY facilities Assists with special projects as assigned Assists with public relations Assist with intakes and discharges Attends and participates in all required meetings Demonstrate the ability to work effectively with other team members Other duties as designated by Program Director Required Job Skills : Oral and Written Communication Skills. Interpersonal Skills Familiarity with various issues including parenting, mental health, domestic violence and chemical dependency/recovery preferred Ability to exercise good judgment, tact, diplomacy and compassion when problem solving, handling conflict or in a crisis situation. Ability to complete record keeping in a timely fashion Ability to complete record keeping accurately and in a timely fashion Ability to get along with diverse personalities and maintain effective working relationships Ability to work well with people of all races, backgrounds and needs Ability to make decisions quickly based on available information and take action Attentive to all aspects of the work environment Knowledgeable of Behavioral Health Concepts and how to incorporate concepts into practical application Ability to work as part of a team and to positively represent the agency in the community Knowledgeable of the stages in recovery for behavioral health and/or chemical dependency Computer knowledge Organizing and coordinating skills Receptionist skills Ability to gather and compile information and prepare reports Additional Responsibilities: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Physically able to reach, push, pull, stoop, stand, walk and lift up to 20 pounds Possess a valid Driver’s License, maintain a safe driving record, and maintain the required minimum vehicle insurance amount for the State of Kentucky, if required for job performance. Maintain CPR/First Aid certification Work Environment: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee occasionally works in outside weather conditions or may be off-site from normal office location. The noise level in the work environment is moderate to loud. Environmental Data and Job Hazards : Hours of work are structured 8.0 hour blocks of time but may be subject to irregular and/or flexible hours based on needs or demands. Position may require duties to include routine or reasonable anticipated tasks or procedures where there is a degree of actual or potential exposure to blood or other infectious materials. All staff are required to participate in yearly trainings regarding infection control and safety in the environment including hazardous waste and blood borne pathogens. All new staff must attend training regarding handling physical, verbal threats, acts of violence or other escalating and potential dangerous situations including when police need to be summoned. All staff must be certified in Cardiopulmonary Resuscitation within one month of employment and maintain the certification, if providing direct care. Powered by JazzHR
