At Health Connect America, we are seeking a compassionate and skilled Substance Use Case Manager to join our dedicated team in making a profound difference in the lives of individuals navigating the challenges of substance use recovery. As a pivotal part of our organization, you will be at the forefront of offering holistic support, guidance, and care coordination to those in need, empowering them to achieve lasting recovery and improved quality of life. What will I be doing? In this position you will help ensure that client’s are consistently receiving medical care, behavioral health services, and access available resources within their communities. Will serve on location in Cedar Recovery Office locations. Will report regularly to leadership to receive support and guidance in providing needed services. Am I a good fit? Do you have the ability to comfortably engage with varied populations in the community via telehealth/phone calls? Are you organized, motivated, and task oriented? Qualifications: Bachelor’s degree REQUIRED with preference given to human service fields or other health related discipline. Experience with case management and work with clients with substance use disorders preferred Health Connect America and its companies are a multi-state, community mental health, behavioral health, family preservation, and therapeutic foster care services provider(s).
