MAT Nurse (RN) Case Manager

Siouxland Community Health Center has 1- Full-time opening for: MAT (RN)Nurse Case Manager 1 - Full-time (Monday- Friday) with one long day until 8:00pm and one 1/2 day. Successful candidate must have solid experience & advanced knowledge of: Minimum of RN license, BSN preferred. Current BLS and Mandatory Reporter certifications. Clinical office and case management experience preferred. Experienced RN with a background in case management who is self-motivated and capable of working with others to improve patient care. Successful candidate must be able to perform primary functions of position: In collaboration with provider, performs initial assessment and intake obtaining social, medical, mental health, substance abuse, and medication history. Provides education and assessment to patients looking to access treatment programs. Offers referral services for patients looking to access treatment programs. Assesses and monitors patients in the induction, stabilization, and maintenance phases of treatment. Provides ongoing management, education, and support of patients in all phases of treatment including; nursing visits, urine toxicology screens, routine labs, medication teaching, monitoring, pill counts, observed dosing, and medication refills. Follow state and federal guidelines in providing care to opioid-dependent patients in collaboration with license, prescribing providers, and pharmacy. Complete appropriate documentation for all patient visits and phone calls in a timely manner, ensuring that all notes are complete and easily understood. May be required to float to the South Sioux City location once MAT services are offered at that location.
