Cayuga Counseling Services, Inc., a non profit human services agency, located in Auburn, NY is seeking a Care Manager Supervisor. The Care Manager Supervisor will be responsible for supervising, coaching and developing of Care Managers to ensure excellent execution of Children's Health Home of Upstate New York (CHHUNY) mission and quality delivery of care management values and principles with engaging, planning, coordinating, facilitating, and transitioning children and families with positive outcomes. Care Manager Supervisor Responsibilities include, but are not limited to: Develop Care Manager talents through competency development in care management and ensure that all the required duties of care management, as implemented by the Care Managers, are met. Communicate and explain organizational goals and strategies and ensure adherence to Agency and CHHUNY policies and procedures. Ensure eligibility of each member through MAPP. Conduct monthly quality checks on child/youth charts (max of 10 per month) to ensure compliance with CHHUNY requirements, address concerns in a timely fashion, and submit bi-annually, at least one month prior to the audit. Ensure that all initial linkages are established and maintained, and Care Managers are advocates for additional services and linkages as appropriate. Articulate clear job expectations, provide timely and frequent feedback through individual weekly supervision. Provide team/group meetings on a regular and consistent basis. Ensure documentation compliance by conducting ongoing monitoring of Tableau reports to monitor due dates and address any areas overdue. Ensure complete and sustainable transition planning is facilitated for youth and family. Serve as liaison to the contracted CHHUNY Administrator on matters directly relating to child and family services and required approvals. Ensure that Care Managers obtain the required CHHUNY training and certifications. Serves on-call on a rotating basis so 24/7 coverge is maintained. Temporarily take on small case load if Care Manager position becomes vacant. Ensure timely submission of billing to fiscal office monthly. Knowledge, Skills and Abilities: Bachelor of Arts or Science Degree in psychology, sociology, human services or related field with two years of relevant experience, or A Licensed Registered Nurse with two years of relevant experience, or A Master of Arts or Science Degree with one year of relevant experience. (Relevant experience includes proficiency in the human services or medical field in the area of mental health, substance abuse, public health, early childhood or established proficiency on an individual applicant basis.) Minimum of 2 years prior supervisory experience in children's mental health, child welfare, juvenile justice, special education or a related public sector human services or behavioral health field working with at-risk children and their families. Become CANS-NY certified within 90 days of hire and all other training within 6 months.
