Position Details Assess needs of patient/family throughout provision of care. Develop, utilize and update a written plan of care specific to the patient in conjunction with the physician's plan of care, including other disciplines as appropriate to patient's needs. Modifies plans as appropriate. Provides skilled nursing care, general health guidance, and teaching on part-time, intermittent basis. Provide the learning tools necessary and actively participate in health teaching of the patient and their families at their level of understanding. Explore with families the resources within the family and/or community to improve their health status and help the family utilize these resources. Recognize and interpret changes in the patient's environmental setting, his/her physical condition and/or emotional state. Report significant changes in these areas to the appropriate person and make appropriate recommendations. Maintain accurate and complete records of nursing visits including observations, cares provided ie. procedures. What You Offer A current Registered Nurse License in the State of Minnesota. CPR Certification is required (certification assistance is available). Have a current, valid Minnesota driver's license and have valid car insurance. Excellent interpersonal skills and ability to work independently. Ability to effectively communicate in English, both verbally and in writing, and act in a positive, professional manner. Must be able to pass background study through the Minnesota Department of Human Services. Work Schedule 64 hours a pay period; 4 Days a week - 8am-4:30pm; Rotate Monday-Thursday Call One Night a Week; Rotate Every 5th to 9th Weekend; Rotate Every 9th Holiday; What We Offer People join Perham Health for the opportunities and stay because of the culture.
