Job Summary: Episcopal Church Home our 131 bed Skilled Nursing Facility has a rare opportunity for a Licensed Social Worker to join our Social Service team. This position will work as a member of the social service team working with a case load of long term care residents. Job Duties: Coordinates social services for the resident including staff and families. Creates resident plan of care and treatment goals related to psychosocial needs. Schedules and attends care conferences for residents on caseload. Completes assessments and MDS charting as required. Serves as a liaison between the resident and the facility. Works with residents on concerns. Coordinates resident utilization of community resources. Works as a member of the interdisciplinary team.
