Reporting to the Regional Director of Quality Care and part of an interdisciplinary team, the MDS Coordinator will assist with supporting Hamlets homes to meet all Health Authority requirements involving MDS including the backssment process, care planning, utilization of the outcomes scores, ensuring the standardized times lines are met, and a regular auditing process is in place for accuracy of coding. The MDS Coordinator will be responsible for supporting team members with MDS education and annual testing as appropriate. Responsibilities Support the homes in the completion of MDS backssment on all residents within established timelines. Understands how the MDS backssment integrates with existing clinical tools and provides leadership in streamlining these processes. Understands and provides critical analysis of data at different levels of aggregation. (Resident, Health Authority, etc. ). Articulates for the team the strengths of MDS as a clinical backssment tool including how CAPS (Clinical backssment Protocols) triggers and care plans enhance quality care. Acts as key contact and liaison with: Health Authority /CIHI/PCC or others as appropriate on RAI data. Keep data for each home which team members completed training and completed annual AIS test. Develops home specific material within the PCC program as required. Audits data prior to CIHI submission. Facilitates response and correction of submission errors with Hamlet.
