As a Tribal Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. CHW's work in a team - based structure and spend the majority of their time in the community engaging directly with members. Interesting in learning to work with medically complex patients who may be experiencing significant addiction and/or behavioral health conditions is important for this role. This position is full - time (40 hours / week) Monday - Friday. Employees are required to have flexibility to work any of our 8 - hour shift schedules during our normal business hours of 8am to 5pm. It may be necessary, given the business need, to work occasional overtime. This position is a field - based position with a home - based office. You will work from home when not in the field. If you are located in Metro area of New Mexico, you will have the flexibility to work remotely* as you take on some tough challenges. Primary Responsibilities: Create a positive experience and relationship with the member Practice cultural sensitivity and cultural competence in daily careLearn and listen to member needs and barriers to help promote self advocatingWork with care coordinator to develop a care plan and set goals for member care Collaborating with clinical team of social aspects that might impact treatment planProactively engage the member to manage their own health and healthcareSupport the member to improve their wellbeing by staying out of the hospital, and attend regular visits to their primary physicianAs needed, help the member engage with mental health and substance use treatmentSupport the member to ensure pick-up of their prescriptionsProvide member education and health literacy on community resources and benefits to encourage self sufficiencyConduct post discharge activities from hospital facility and support connection to social servicesSupport member to engage in work or volunteer activities, if desired, and develop stronger social supports through deeper connections with friends, family, and their communityPartner with care team (community, providers, internal staff)Knowledge and continued learning of community cultures and valuesConduct Health Risk Assessments (HRA) You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Years of post-high school education can be substituted/is equivalent to years of experience Required Qualifications: 18 years of age or olderKnowledge of culture and values of the communityFamiliarity with the resources available in the communityProven ability to navigate a PC to open applications, send emails, and conduct data entryAbility to travel locally up to 50% of the timeLived in New Mexico area for 2 yearsCurrently reside in New Mexico Designated workspace inside the home with access to high - speed internet availability Preferred Qualifications: Licensed Practical Nurse (LPN), Certified Nursing Assistant / Home Health Aide, Certified Medical AssistantCHR/CHW AccreditationKnowledge and / or experience with behavioral health or substance use disorders2+ years of field - based experienceCare management experienceExperience working in Managed CareKnowledge and Training on SDOH (Social Determinants of Health)Knowledge of Medicaid and / or Medicare populationSpeak one or more Native American languages *All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy At UnitedHealth Group, our mission is to help people live healthier lives and make the health system work better for everyone.
