Job description The VNA & Hospice of the Southwest Region is looking for a per diem Speech Language Pathologist for our Kids on the Move program in Rutland County. Under the direction of the Clinical Manager of Kids on the Move, evaluates, plans, and administers medically prescribed or educationally relevant speech therapy treatment to clients in their homes or students in their respective educational environment. In addition, this position is responsible for clinical supervision of students and SLP's in CFY year and students. Key responsibilities include (but are not limited to): Actively monitors and amends patient visit schedule to maximize productivity and patient need. Takes action to account for new patients and changes in existing patient schedule, including but not limited to, calling patient to reschedule visits, reshuffling patient load to meet new demands, communicating recommended changes to supervisor, etc. Performs a comprehensive (OASIS and SLP) assessments of assigned patients including, but not limited to psychosocial, educational, safety and risk factor parameters, as evidenced by clinical documentation. Develops a plan of care to promote optimal functional independence and safety as documented by clinical record in accordance with federal and state regulations, organizational and departmental policies and standards of care and APTA guidelines. Plans for and provides skilled speech-language therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, etc. assessing the patient's status and progress on each visit and ensuring quality of care as documented in the clinical record. As Care Manager and/or team member, coordinates patient care with other clinical team members, including but not limited to, physicians, nurses, care givers, other rehab services and students, ensuring optimal patient care and communication as noted in the clinical record. Accurately and completely performs functions as related to school-based services, including, but not limited to, evaluations, documentation, development of Individual Educational Plan IEP's, participation in meetings, and provision of direct and consultative services. Fills out and completes insurance paperwork in a timely manner and in accordance with applicable regulations. Actively provides services in accordance with Vermont State Department of Health, including but not limited to, clinic consultations, follow-ups, seating and mobility assessments, etc. Supervises and directs clinical activities of SLP students and home care aides in accordance with federal and state regulations, department and organizational policies and standards of care and ASHA guidelines. Completes and coordinates required paperwork in accordance with federal and state regulations, organizational and departmental policies, standards of care and ASHA guidelines to ensure that documentation reflects care provided and the outcomes achieved as evidenced in the clinical record. Completes required paperwork to ensure charging and billing processes are accurately executed, including, but not limited to, documents that support charges entered, care within insurance coverage parameters, required insurance forms completed etc. Takes action to address inconsistencies in insurance coverage and desired plan of care requirements. Notifies supervisor of issues as they arise in a timely manner. Participates in in-service training programs for our customers including, but not limited to Rehabilitation, Medical, Nursing staff, volunteers, community groups, etc. Communicates changes in patient care schedules to scheduler and supervisor in a timely manner. Completes OASIS time points in accordance with agency, State and Federal guidelines, regulations and policies. Accurately and completely fills out a variety of Agency, State and Federal forms, including but not limited to, HHABN, Medicare Notice of Non-Coverage, etc. in a timely manner. As necessary, assesses patients' needs for HCA services, develops and coordinates POC with patient/care giver, and provides on-going supervision. Coordinates safe patient discharges and/or transfers (or upon knowledge of) and ensures all appropriate paperwork and patient information is processed. As necessary, transmits patient incidents reports (infections, injuries, etc) to QI Specialist in a timely manner. Serve as a Home Care ST as required Qualifications: Master's degree in Speech Language Pathology required Current Vermont State SLP license required Active enrollment in clinical fellowship year or successful completion of certificate of clinical competency required Valid Driver's License Experience: Two years of clinical Speech Language Pathology experience required
