Philadelphia FIGHT HIV Medical Case Manager Job Description Hours: M-F 9:00 to 5:00 Department: Client Services Location: Jonathan Lax Treatment Center Supervisor: Director of Medical Case Management Full-Time, 40 hours per week; Exempt Philadelphia FIGHT is a leading Federally Qualified Health Center with four clinical sites, providing access to state-of-the art, culturally competent primary care, advocacy, social services, consumer education, and outreach for all patients in need, regardless of HIV status or insurance status. Each clinic has a unique focus, these include: primary care for People Living with HIV, primary care for individuals returning from incarceration, primary care for pediatrics, adolescent, and young adults, primary care for people experiencing homelessness, primary care for injection drug users, gender affirming care, dental and behavioral health. Providing patients with case management services is a vital component in providing to their medical care. Job Summary The Medical Case Manager position, based out of Philadelphia FIGHT's Jonathan Lax Treatment Center, will provide long and short-term medical case management services, therapeutic support, treatment adherence and benefits coordination to adult patients living with HIV. This includes connecting clients to necessary resources and public benefits such as Medicaid/health insurance, cash assistance, food assistance (SNAP), housing rebate programs, medication assistance programs, and additional resources such as shelter/housing, food banks, substance treatment programs, behavioral health programs, and other necessary resources that will enhance clients' lives. The HIV Medical Case Manager position will work in an interdisciplinary team setting with medical and behavioral health providers to provide medical case management to adults living with HIV and receiving medical care at Philadelphia FIGHT. Tasks and Responsibilities Maintain a caseload of clients living with HIV/AIDS. Complete a biopsychosocial assessment and service care goals with each client at the time of intake. Monitor and update service care goals and assessments quarterly in coordination with interdisciplinary team. Maintain contact with clients based on program need, client need, and service care goals. Documentation of adherence to medical appointments and medical care plan, including HIV medication adherence. Provide trauma-informed therapeutic support in collaboration with interdisciplinary team, including crisis situations. Identify appropriate resources and external support systems to address client needs including advocating and negotiating linkages to resources. Available to provide home visits and accompany clients to medical appointments and outside treatment facilities as appropriate. Maintain accurate and timely documentation of all client contacts (using D.A.P. format), forms, and applications in EHR software. Maintain confidentiality standards in accordance with applicable laws and regulations, specifically including the Health Insurance Portability and Accountability Act (HIPAA) and the Confidentiality of HIV Related Information Act (Act 148). Participate in on-call rotation during regular office hours (Mon-Fri 9-5). Perform other duties as assigned. Contribute as a team member and share in the responsibilities required to maintain operations and serve the mission of Philadelphia FIGHT. This includes, but is not limited to, obtaining training, participating in events, flexibility in scheduling, covering the commitments of coworkers when they are unable, and other duties as assigned. Qualifications Minimum of a bachelor's degree in social work (or related field) from an accredited college is required. MSW preferred. Knowledge and familiarity with local social services resources. Ability and desire to work productively with highly stigmatized and diverse populations using a strength-based framework. Excellent interpersonal skills and ability to work collaboratively with clinicians and patients. Ability to exercise crisis interventions. Bilingual (Spanish/English) is highly desired. Ability to work from home with WiFi / Computer Access as needed. Required Screenings Must complete Philadelphia FIGHT Credentialing and Privileging to provide direct services with Federally Qualified Health Centers. Philadelphia FIGHT requires proof of completed COVID-19 vaccination series for all new employees hired. Medical and religious exemptions will be honored consistent with EEOC regulation. Twenty hours of professional education and training will be required on an annual basis as described by AACO. The AACO core training series must be completed within 12 months of hire. Physical Demands and Mental Demands Ability to work under pressure and be creative and analytical. Ability to continuously stand, walk or sit. Ability to work at a computer screen for long periods of time. Ability to bend, climb stairs frequently. Ability to lift or push up to 10 pounds infrequently. Ability to squat occasionally. Availability for occasional work outside of regular office hours Work Environment Generally works in a clinical office environment but may occasionally be required to perform job duties outside of the typical clinical setting.
