Home Again Assisted Living of Columbus is currently looking for a Community Director to join our team! This position is full time (40 hours per week) & requires to be on site. The Community Director is responsible for assisting the Medical Director and the Director of Operations in the daily tasks associated in running the community. Assisting in the provision of quality care of the residents of Home Again Assisted Living by carrying out the responsibilities and duties set in the description. The Community Director acts as an advocate, encourages and supports choice, independence and individuality and provides confidentiality and respect of all residents. What your day to day looks like: Maintain inventory and all activities related to ordering, supplies, and changing oxygen related equipment for Residents Daily Medication program which includes medication reorders and follow up, review physician orders, communicate with pharmacy, set up new medications and discontinue when needed. Medication Administration Record oversight including all vital tracking, medication initials, and actual distribution of medications as directed by the Medical Director. Medication Cart which includes making sure they are clean, fully stocked, and properly set up per Medication Director. Chart Oversight which includes monitoring Resident Assistant charting, follow ups, and appropriateness of charting. Falls Follow which includes documentation, physician notification, charting, and any other related items. Doctor Appointment coordination and follow up. This also includes making sure documentation is received after visit and all orders are complete. Resident medical needs assessment and monitoring as assigned by Medical Director. Medication training of new Resident Assistants; review the medication cart, medication administration records, process to follow, and guidelines and expectations. Pre and post move in assessments in conjunction with Community Director Resident vitals tracking and completion, along with physician notification when needed. Attend staff meetings and communicate any health or medical or training information as advised by Medical Director Change in Care and ISP updates - work with CD and Medical Director on any change needing a rate adjustment Employee Injury triage when available Manager On-Call Rotation Day to Day Resident and Family Interactions which include requests, appointment coordination, and meetings/calls. Day to Day medical communication assistance which includes interaction with clinics, doctors, and nurses Annual resident reviews (joint task with nursing team), Tracking and completing reviews Other duties as assigned This position also is required to assist as a Resident Assistant when requested.
