ADMISSIONS NURSE Friday - Sunday8:00 AM - 8:00 PM$1000 Sign on BonusWeekend Shift Diff Pay Position Summary: The Admissions Nurse is responsible for assuring that Hospice of South Texas programs are effectively represented to patients, families, and others who have an interest in hospice through pre-assessments, information sharing and/or admissions in a patient's home or in a health care facility. This position works with the intake team, referring physicians and other referral sources to promote the hospice philosophy of care. Also required to make as needed nursing visits and triage calls. Work occasional holidays. Qualifications: Education : Graduate of an NLN accredited nursing program; BSN or CHPN preferred. Experience : Minimum of one year of nursing experience required; intake, home care, hospice and/or oncology nursing experience preferred. License/Certification : Registered Nurse licensure by the State of Texas; valid driver's license with an automobile that is insured in accordance with state and/or agency requirements and is in good working order.
