Medical Social Worker JOB DESCRIPTION Summary/Objective To review Social Services Assistant's records and work with the care team in eliminating barriers which preclude the client/family from achieving maximum benefit of home care services rendered, including but not limited to, accessing community resources and agencies which can best provide problem resolution. Performance Responsibilities: Utilizes established lines of authority and communications: Reviews care/records and signs name and credential on records for Social Services Assistant work. Communicate clear, concise, relevant, non-judgmental information. Maintain open lines of communication with all staff. Utilizes effective problem-solving/conflict resolution skills. Promotes supportive group process. Utilizes chain of command for problem resolution. Demonstrates personal and professional accountability: Demonstrates knowledge of applicable social work standards in accordance with Medicare Conditions of Participation. Accepts responsibility for personal and professional accountability for own practice. Complies with and enforces all agency policies and procedures. Demonstrates effective priority setting and time management skills. Performs duties and appropriate LCSW supervision (bi-monthly and as needed chart reviews by LCSW for appropriate interventions, documentation and co-signature on notes). Presents a positive image of the agency. Demonstrates an attitude of caring when dealing with clients, families, and staff. Complies with ethical conduct and displays competency in performing assessments and treatment techniques according to professional standards. Complies with timely, accurate, and appropriate documentation of care and communications pertaining to patient care. Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Identifies social and emotional problems that impede patient recovery. Applies appropriate goals and interventions to resolve identified problems and communicates identified social service plan including frequency, treatment and goals to the clinical supervisor and primary nurse to facilitate obtaining orders to assure compliance with care given Clinical and administrative documentation adheres to agency, professional, and regulatory guidelines for timeliness and content. Initiates and completes referrals to community resources as needed. Attends required staff meetings and case conferences Contributes to the coordination of services through effective and timely communication with other members of the health care team. Facilitates appropriate supervision as required by COP/state regulations (chart reviews by LCSW for appropriate interventions, documentation, and co-signature on notes). Maintains the caseload and agency productivity standards. Responds to referrals to social work in a timely manner and documents and communicates case according to the agency standards. Maintains Clients' self-esteem, privacy, and confidentiality of personal information. Assures client rights are protected and that all clients/caregivers are made aware of client rights and responsibilities. Initiates emergency procedures as appropriate. Position Qualifications Licensed Clinical Social Worker per Colorado Law Prior social work experience Language Skills Ability to read, write, and present information to small groups. Strong communication and teamwork skills . Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee must be able to sit or stand for extended periods of time. Specific vision abilities required by this job include close vision, distance, vision, color vision, peripheral vision, depth perception and ability to adjust focus. Specific hearing abilities required by this job include ability to perceive the nature of sounds at normal speaking levels with or without correction. Must be able to receive detailed information through oral communication, and to make the discriminations in sounds . Possible Job Modifications/Accommodations: May sit on stool for countertop work if standing is a limitation. May dictate information if writing is a limitation. May use a reaching tool or step stool, if reaching overhead is a limitation.
