Katy Trail Community Health is seeking a LPN/Phlebotomist is a member of a health care team, which includes, a provider, a behavioral health consultant, a licensed practice nurse (LPN), a registered medical assistant (RMA)/certified medical assistant (CMA), a care coordinator, and a patient service representative (PSR) that have been assigned a panel of patients. Under immediate supervision, supports providers (physicians, nurse practitioners, and physician assistants) in provision of patient care; includes recording vital signs, discussing chief health concerns, performing testing, and administering injections; assists with procedures and documentation.** The COVID-19 vaccination is mandated for all Katy Trail Community Health employees. Responsibilities and Essential Functions: Phlebotomist (Under the supervision of a physician/RN/NP/PA/Director of Clinical Services): Properly identify patient and verifies provider's orders prior to blood draw or other testing.Assures lab requisition contains ICD-9 code and checks with nurse/provider as needed.Accurately draws blood samples, labels specimens, and thoroughly completes lab requisition.Completes ABN form when needed for Medicare patients and has patient's sign when appropriate.Keeps up to date with phlebotomy procedures and what specimen container is required for each test.Venous and capillary samples are obtained with minimal trauma to patient.Observes Universal Precautions.Maintains clean lab area, and stocks appropriately.Maintains any appropriate logs or manifests, assuring all labs drawn or tests performed are received in a timely manner, referring all abnormal stats to the appropriate nurse/provider.Performs laboratory tests including but not limited to urinalysis dipstick, urine pregnancy tests, and hematocrits.Checks nursing line multiple times per day, filling out phone message form, returning calls as needed for clarification, and giving paperwork to Medical Records Clerk/nursing staff as needed.Monitor and log refrigerator temperatures daily in lab area.Monitor and maintain logs for all eyewash stations.Assists Patient Assistance Coordinator with the Referral Process. LPN ( Under the supervision of a physician): Correctly assesses patient's vital signs, anthropometric measurements, and charts them accurately.Accurately obtains medical/social/family /medication history including over the counter medications and allergies/reactions to medications. Also assists provider in assessing a patient's progress toward their goals and barriers that may be obstacles to achieving their goals.Obtains consent forms as needed to obtain medical records from outside providers.Performs laboratory tests including but not limited to urinalysis dipstick, urine pregnancy tests, and hematocrits.Obtains samples for lab tests through venipuncture. Monitors medical supplies and stock medications, stock treatment rooms, and orders supplies to maintain supply levels.Triages patient phone calls for minor health problems and refers complicated illness to nurse practitioners, and physicians.Accurately performs a variety of testing, including but not limited to audiometry, vision testing, peak flow and lung function tests, EKGs.Safely operates centrifuges and autoclave.Monitors and logs refrigerator temperatures daily.Logs all laboratory testing manually or electronically as needed, reviews laboratory results when returned from outside labs, checks them off as received, and refers all abnormal results to the correct provider.Accurately administers and charts medications orally or by intramuscular or intradermal injections to patients through the life span.Completes pre-visit planning on all patients scheduled to be seen by their respective care team including a review of the medical chart to insure that all lab tests, diagnostic studies and specialty reports have been received in advance of the patient's scheduled appointment..Follows standing orders as defined by the Chief Medical Officer including prescription refills.Safely administers immunizations as ordered by the provider and/or as defined by standing orders.Assists provider in completing structured templates for narrative progress notes.Assists provider in offering patient education/handouts and documenting accordingly.Assisting provider to follow evidence based guidelines for chronic diseases and review flow sheet for chronic disease compliance. Order testing for chronic disease as established in standing orders.Assist provider to identify patients that may benefit from care coordination and behavioral health. Minimum Qualifications: Possess high school diploma or equivalent.Possess licensed practical nurse license.Possess current CPR certificationPossess basic keyboard skills and computer knowledge.Bilingual abilities encouraged.Must demonstrate accuracy, proficiency and efficiency from approved training program.Must be able to see, hear, read, write, kneel, stand, sit and lift up to 50 lbs.Must be able to handle stressful situations.Possess cognitive skills necessary to understand medical terminology.
