Summary The Shelter Case Manager should provide housing assistance for guests of the shelter utilizing housing first and harm reduction theories, along with non-authoritarian problem resolution techniques. All employees are expected to learn and exhibit the core values of Catholic Charities Acadiana as defined in the employee handbook and on the CCA website. Duties will include but are not limited to the following: Provide service coordination for shelter guests. Assist shelter guests with the development and implementation of service plans, including housing placement plan and medical care, along with any other needed services. Assist shelter guests with obtaining and maintaining mainstream benefits Maintain comprehensive and complete records for each shelter guest. Document all services provided in Service Point within 24 hours of service delivery Provide shelter guest advocacy and referrals to outside agencies Ensure cleanliness of shelter guest rooms Provide life skills training to shelter guests as requested Coordinate service plans with other social service providers Attend scheduled service coordination staffing meetings Provide transportation and/or bus vouchers Monitor medication of shelter guests Maintain office area and facility in clean and orderly fashion Provide crisis prevention/intervention as needed Communicate regularly with Supervisor Be courteous and polite at all times Attend all training assigned by supervisor Any other duties assigned by supervisor Job Qualifications Minimum of a high school diploma/GED, with two years of service coordination or related social service experience required. Bachelor's degree preferred. Knowledge of homelessness and housing programs helpful Experience providing services for individuals and families experiencing homelessness Exceptional problem resolution/ de-escalation skills Exceptional oral and written communication skills Valid LA driver's license and an insured motor vehicle in which to transport clients Job Specifications Medium: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects 100% of the time.
