Clinical Manager, Home Health Job #105-XXXX Jefferson Healthcare ( www.jeffersonhealthcare.org ) is seeking a Nursing Manager to lead the Home Health program of our Home Health & Hospice department . Our amazing staff in Home Health & Hospice (HHH) work independently to provide our community the highest quality of care. They and their work is critical to our mission to provide the highest quality care to the community and we are looking for a leader who is a champion of that philosophy. Who are you a BSN or MSN prepared RN you have 5 years of clinical experience in home health or acute setting a nurse leader with 3+ years of supervisory experience adaptable, a self-starter, and motivated by being part of an incredible team have a desire to serve the community and provide excellent care committed to excellent care and customer service have a desire to manage the Home Health program of our Home Health & Hospice department want to contribute current knowledge to and grow in operations, administrative and personnel responsibilities want to lead a multidisciplinary team of clinical and support services staff that includes full-time, part-time and per diem staff members If this sounds like you, we look forward to meeting you! Promotes continuity of care to ensure that patient needs and expected outcomes are met. Must be capable of functioning as a member of the team. This includes supporting, delegating and supervising the delivery of medical care, operational performance and quality of service. This is a full time, exempt position.
