JFS of WNY is accepting applications for a full-time Health Home Care Coordinator in our Niagara Falls office. Do you have a case management background and the ability to work to resolve problems that may prevent access to needed medical services and advocate on the behalf of clients? WHAT YOU'LL DO In working with your clients, you'll be promoting client rights and access to social service, social security, mental health, and other support services. As part of the Care Coordination team, you'll manage all aspects of care for Health Home consumers. These responsibilities include tracking and arranging appointments, working in partnership with treatment providers, offering consumer advocacy and education, and coordinating other aspects of the consumer's community services. Equally important are keeping accurate documentation, maintaining case notes, and completing (and monitoring!) all required forms. For this reason, you must not only have proficient communication skills but must also have excellent writing skills and attention to detail. WHAT YOU'LL BRING Minimum of an Associate's degree in a relevant discipline is required; Bachelor's degree preferred Familiarity of Niagara County services and resources Experience in care coordination with community-based client services or discharge planning desirable Ability to effectively provide case management services, responding supportively, and with persistence, to client families Familiarity with community linkage resources helpful Willingness to work within the community and make home visits Experience with electronic medical records a plus Proficient verbal & written communication skills for interactions and for documentation Familiarity with and appreciation of issues related to people with diverse cultures and languages WHY JOIN JFS? We also have a reduced 35 hour work week with an early close on Fridays! This position services Niagara County with a focus on Niagara Falls. Since this position requires work in the community, you must have a reliable vehicle, a clean NYS driver's license and required liability insurance levels of 100/300k.
