The Visiting Nurse Association of Florida (VNA) is a 45 year old home health agency that is growing and looking to hire Physical Therapists in the West Pasco, Florida. You will be responsible for completing an initial assessment of the patient to determine home PT needs, developing and implementing a treatment plan for each patient while using VNA of Florida's case management model.
