Emergency Department Discharge Follow Up Nurse

The ED staff position is an integral part of the population health management team that is responsible for contacting and educating patients that have used an Emergency Department for their care. This patient contact is an opportunity for the staff and the patient to a develop a plan to prevent future Emergency Department visits. This approach is aimed at improving the knowledge of the patient related to their disease process, provide information about obtaining a visit with the patients' primary care provider, how to use the after-hours number and when it is appropriate to use the Emergency Department for care. The staff will coordinate the process, working collaboratively with physicians and integrated team members of Health Ministries Clinic to ensure any follow up care that is needed is provided. This may include scheduling patient follow up appointments. This staff will work with completing care gaps and completes a patient survey for this patient population. Education and Experience: Preferred Qualifications: · Unrestricted Kansas Nursing License· 1+ years' nursing experience, clinic or public health nursing strongly preferred· 2+ years' experience in care management, medical assistant.· Knowledge of public health preferredRequired Qualifications:· High School Diploma· Strong organizational, communication, and problem-solving skills· Excellent internal and external customer service. Key duties and responsibilities: · Review of the medications, supplements, and vitamins currently verbalized by the patient.· Discuss coordination of care the patient is currently receiving from other healthcare providers.· Education of patient regarding after-hours contact information, educate on emergent needs, i.e., what is emergent and what is non-emergent.· Mail educational information to patient.· Complete patient survey for patient population.· Review of patient's functional ability and level of safety (for example, the risk of falling at home), including any cognitive impairment.· Discussions of referrals to other appropriate health education or preventive counseling services that may help the patient minimize or treat potential health risks.· Schedule follow up appointments with providers.· Communicate and collaborate with providers, medical assistants, and other healthcare professionals to deliver optimum primary care· Create and contribute to patients' electronic medical records to ensure accuracy and completeness· Respond appropriately to inquiries, concerns and complaints from patients, parents, or caregivers· Participate in quality improvement efforts· Follow clinic policies and procedures Key competencies: · Ability to relate to the public with compassion regardless of ethnic, religious, or economic status· Telephone skills and knowledge of customer service skills· Computer skills· Oral and written communication skills· Ability to work within a team and independently· Critical thinking skills· Knowledge of appropriate delegation· Ability to handle difficult situations involving patients, physicians, or others in a professional manner Physical Requirements and Working Conditions: · Environmentally controlled medical office· Interaction with a diverse population including team members, providers, patients, insurance companies and other members of the public.· Frequent sitting, occasional standing, walking, grasping, carrying, reaching, bending, stooping.· Occasional lifting, carrying, pushing, and pulling up to 50 pounds, with assistance if needed· Frequent use of computer, keyboard, copy and fax machine and phone ABOUT HMC: Health Ministries Clinic is an FQHC that provides care to all members of the community regardless of socioeconomic or insurance status.
