Comfort Home Care is in search of Per-Diem Registered Nurse / Home Health in Middlesex North East and the surrounding areas; Acton, Concord, Littleton and Maynard. Our nurses visit a diverse population, mostly with mental health diagnoses, performing nursing assessments, communicating with other providers, and maintaining patients in the community as independently as possible.Comfort Home Care is a supportive agency offering ongoing Educational Opportunities, Health/Dental/Vision, 401k, Accident and Critical Illness Insurance, and much more. Duties and Responsibilities: Comfort Home Care is seeking a Massachusetts licensed Registered Nurse / Home Health that can: Provides initial and ongoing assessments to identify the client's medical needs, level of functioning and ability to communicate and learn. Initiates the plan of care, necessary revisions, and performs OASIS assessments at appropriate time points when applicable. Initiates and performs appropriate nursing interventions. Coordinates services, informs the physician and other personnel of changes in the client's condition and needs. Collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
