Assistant care manager- Education Care and Health Services- Bromley Are you a passionate, dedicated Assistant Care Manager looking for your next role 4Recruitment Services are recruiting a Assistant Care Manager to join the Education Care and Health Services based in Bromley. Umbrella Pay- £23.96 per hour8.30am-17.00pm 35 hours per week The role: To provide practical support for Adults and Carers. Undertake backssments of Adults and Carers needs. Create and update care plans for service users accordingly. Ensure Service Users have access to suitable equipment at home, checking after installation Promote independence. To work closely with qualified or registered workers in order to provide the best support for carers and service users. Work within the departments’ budgetary constraints to achieve best value by providing creative cost-effective services. Monitor and review support plans and the effectiveness of services provided for Adults and Carers. To assist in induction of all new staff. To ensure all parties involved are informed of changes to any support plans. To be flexible and willing to work across all teams in the division. Be in the know and respond to any change in procedures. Requirements: Relevant experience A qualification in advice and guidanceor working towards Knowledge of procedures, services and resources. Ability to work closely with registered workers in order to obtain the standards of care. Experience of case management. An Enhanced Adult and Child DBS registred to the update service.
