Care Coordinator required for seniors' living facility If you are a Registered Nurse or a Licensed Practical Nurse looking for a position in a dependable field with one of the most dynamic employers in Southern Alberta, keep reading. Green Acres Foundation is looking for someone with at least 3 years' experience in nursing who has a flexible, willing, and positive attitude. The right applicant will have successfully completed an approved nursing program and have proof of an active registration with CARNA or CLPNA. A current satisfactory criminal record check is required, and related nursing experience in geriatric care is an asset. About the position: This is a Temporary Full-Time position. Must be available to work weekdays, weekends, and holidays. Shifts to cover in this position are: 6am to 2pm, 2pm to 10pm, and/or 10pm to 6am. Basic Responsibilities of the position include: Oversee nursing and personal care services in a lodge setting. Provide supervision and leadership to care staff in a manner that motivates, guides, and directs employees, promotes staff participation, teamwork, and positive relations. Participate in the development and coordination of a comprehensive resident centered plan of care. Work collaboratively with Home Care and other members of the inter-disciplinary team. Assist with the admission, discharge, and transfer processes for all residents receiving care services in accordance with care plans and as requested by Site Management. Maintain effective relationships with the residents, families, and AHS. Perform in scope nursing duties as needed and as directed by Site Management. Perform other duties as assigned.
