Role Overview - Health and Wellbeing Practitioner HMP The Mount: In this dynamic role, you'll deliver client-focused treatment, utilizing harm reduction and abstinence-based methods as needed. You'll manage cases, provide harm reduction advice, conduct backssments, and create care plans. You'll lead 1:1 and group sessions on substance misuse and low level mental health topics, facilitating smooth transitions from prison to the community for release preparation as well as make case note entries and progress reports. Close collaboration with various services, such as Transitional, Housing, Employment, Gym, Healthcare, Probation, and external Drugs Intervention Teams, is integral to preparing for release.
