Responsibilities Conducts assessment and evaluation of behavioral health conditions by means of diagnostic and screening tools, clinical interviews and consultation with the medical team. Conducts diagnostic evaluation and documents assessment in medical record. Conducts diagnostic evaluation and confers diagnosis as appropriate. Develops an individualized treatment plan of care. Collaborates with patient, family, and other colleagues to develop a treatment plan of care. Develops an individualized plan based on age, social and cultural considerations, condition, and other relevant needs. Maintains required, appropriate, and timely communication and correspondence with treatment authorization and payer sources in a manner which promotes efficient and effective delivery of clinical services. Qualifications Required Qualifications: Masters or Doctoral Degree in Social Work or Psychology Licensed Social Worker (LICSW) or Doctoral Level Licensed Psychologist (LP) 1 years post licensure Preferred Qualifications: 5 years post LICSW / LP licensure Previous experience completing diagnostic evaluations, and providing individual, couples, family counseling for children and adults Healthcare experience Knowledge of health conditions and common co-morbidities Knowledge of the continuum of behavioral health care services (i.e. within surrounding communities) Experience with Epic Click Here to Apply!
