Job Title: Clinic Registered Nurse Department: Cayuga Center for Metabolic and Bariatric Surgery and Cayuga Center for Healthy Living Reports to : Practice Manager of Cayuga Center for Metabolic and Bariatric Surgery and Cayuga Center for Healthy Living Job Summary : This position provides direct care and coordination for metabolic/bariatric surgery patients and non-surgical patients addressing various chronic health issues through lifestyle modification. Works with the team to assure safe and consistent patient care delivery. Applicant should have strong clinic nursing skills, analytical, problem solving, critical thinking, organization and decision making skills. The RN must possess the ability to function independently and have the flexibility and personal integrity to work in partnership with the CHS network partners as needed. Applicant should also have strong attention to detail with the ability to prioritize, coordinate care and simultaneously maintain multiple projects with high level of quality and productivity is a must. Additional position requirements include current NYS RN licensure. One to two years of experience in an inpatient or outpatient medical or surgical practice along with strong written and verbal communication skills required. Previous experience in surgery, metabolic disease, quality improvement or project development is highly desirable. Job Responsibilities include : Promote the mission, vision, and values of Cayuga Medical Center and Cayuga Health Systems. Work closely with providers to manage pre and post-operative bariatric surgery care, including triage calls, coordinating pre-op testing and routine post-op visits/phone calls within scope of practice. Appropriate documentation within scope of practice in Medent EMR system. Perform patient follow-up care management and facilitate patient engagement with medical direction, in collaboration with patient's provider and extended care team. When directed by provider, monitor patient adherence to plan of care and progress toward goals, to provide nursing support and facilitate changes as needed. Cultivate and support inpatient and outpatient care teams to ensure appropriate management of the Bariatric Surgical patient. Serve as a point of contact and informational resource for patients, family, and care teams. Work with support staff to ensure proper medication management and adherence to follow-up appointments. Perform "nurse visits" to coach and provide follow-up support to patients Recognizes and respond to opportunities for improvement in patient care. Initiate, perform and track prior authorizations for patients.
